- If your hospital's general consent allows UNencrypted data transfer to third parties, use this for all cases.
- Else, if the infant needs to be recorded with date of birth (e.g. for future follow-up), use the SwissNeoNet informed consent.
- In all other cases, use your hospital's general consent, if it allows encrypted data transfer to third parties.

As of 01.01.2015, a signed informed consent is required for each child:
- if date of birth is collected: informed consent must allow non-encrypted data transfer.
- if no date of birth is collected: informed consent must allow encrypted data transfer.
CHUV, HFR and Geneva: use your hospital's general consent. All other clinics: contact us (see below).
